In adult men, Proscar is used to decrease an enlarged prostate (also known as benign prostatic hyperplasia or BPH). It can be used alone or in combination with other medications to alleviate BPH symptoms and possibly avoid surgery. Proscar (Finasteride) may help with BPH symptoms by reducing the need to urinate, improving urine flow with less straining, reducing the sense that the bladder isn't empty, and reducing overnight urination.
This medication works by lowering the quantity of DHT, a natural body hormone that drives prostate growth. Proscar isn't approved for prostate cancer prevention. It may raise the risk of acquiring a very dangerous form of prostate cancer by a small amount.

In fact, a recent review of studies on male sexuality published by the American Society of Reproductive Medicine (ASRM) in April 2005 found that patients who received finasteride, the active ingredient in Proscar, were more likely to suffer from sexual dysfunction compared to patients on placebo. The authors of this study concluded that "patients who received finasteride, who were unable to maintain a penile erection for satisfactory sexual performance, had significantly worse erectile function than those who received placebo."
According to the authors, "the evidence for the use of finasteride in patients with erectile dysfunction was relatively weak. The only clinical study that had evidence of benefit was the study of Pfizer Inc, a major pharmaceutical company that manufactures Proscar. The authors concluded that the effect of finasteride was modest, with no effect on sexual function."
As mentioned above, the researchers did not find evidence that finasteride improved libido or erectile function. They noted that some patients who received finasteride reported improved sexual function, but the effects did not appear to be dose-dependent.
